BEFORE YOU LEAVE FOR SPRING BREAK!!

Please let us know of your plans to return to MLA or not for the next school year by filling out this form and returning to the school office BEFORE March 17th. We need to hear from you before Spring Break begins in order to reserve your spot for next year!! Please indicate whether or not your student(s) will return to MLA next year by: (1) circling YES or NO below, (2) signing the form, and (3) returning it to the school office. Additionally, please inform us of any siblings who you plan to enroll by completing the “New Siblings Only”
section at the bottom of the form. Don’t forget to include their date of birth AND grade they will be entering!
